Checklist For Volunteers

Here is a list of all the documents you will need to gather to take with you when you go and apply for your visa.

Please note some of these are very important and you must not travel without them or your visa may be denied. Others are useful for you to carry as they may be asked for.

You must have a passport valid for no less than two years from your time of departure.

Nonimmigrant visa application form. Essential for application for US visa. Edumentor will help you get this.

Very Important. This is supplied by your host school. Edumentor will help your school with this.

Very Important. Males Only. Edumentor will help you get this.

Very important. Take 4 of these and remember to ask for US Size Passport Photos

Conference Letter

Important. This is supplied by your host school. It documents the schools link to the world wide SDA organization. Important as you are applying for a religious workers visa.

Baptismal Certificate

Important as this proves your membership to the SDA Church and the World Wide Church at large.

Adventist Volunteer Application Form

This will be sent to you from SPD Volunteer department. Fill it in and fax it back to them. Hayley will work with you on this.

Health Certificate

This will be sent from SPD Volunteer Dept. You will need to get a Doctors medical certificate before you fill this out. Fill it out and fax it back to Hayley at SPD.

Adventist Volunteer Service Plan Request For Insurance Cover.

This will come from SPD. Print it out, fill it in and fax it back to them. This is important.

IT IS A GOOD IDEA TO PUT ALL THESE DOCUMENTS IN A CLEAR FILE SO YOU CAN SEE WHAT YOU HAVE AND FIND THEM IF YOU ARE ASKED FOR THEM.

Also add to your file a Police Clearance and your birth certificate. You may not be asked for these, but it is a good idea to have them.
